Submission Form

For a pdf copy of the submission form used for manual submissions, please click on Submission Form. The pdf copy has two forms per page.

Print out the form, fill it out and take it to the gallery. It may be left in the wooden box on the sitter's desk.

Do NOT use the form if you are submitting your title card information electronically.
